CCM is for patients with two or more chronic conditions. You receive monthly check-ins, medication reviews, and coordination with your doctors to help manage your overall health.

RPM uses connected medical devices—like a blood pressure cuff, pulse ox, or weight scale—to send your readings to your VirtuMedex care team. If anything looks concerning, we contact you right away.

RTM tracks non-vital data such as pain, medications adherance, or therapy progress. It’s ideal for patients in pain management, or respiratory rehabilitation, or those having difficulties managing their medications.

TCM supports you post hospital or rehab discharge by ensuring you understand your medications, understand how to manage your condition at home, have everything needed to safely stay at home, schedule follow-up appointments, and avoid unnecessary readmissions.

If your provider recommends Remote Patient Monitoring (RPM), the service will be ordered based on your specific diagnosis, and the appropriate medical devices will be shipped directly to your home within three business days.
Once the devices arrive, they will be pre-configured, activated, and ready to use. As soon as you begin taking your vital signs, your readings will automatically appear on our secure monitoring dashboard.
This allows our clinical team to identify abnormal results early, intervene promptly, and help prevent avoidable hospitalizations by addressing potential issues before they become emergencies.

VirtuMedex partners with FDA-approved medical device platforms. All data is transmitted securely and automatically to the provider dashboard.

The Medicare GLP-1 Bridge is a temporary CMS program that helps eligible Medicare beneficiaries access certain GLP-1 medications for weight management. The program provides coverage outside the standard Medicare Part D process for patients who meet specific medical and insurance requirements. It aims to improve access while longer-term Medicare GLP-1 coverage options develop. Participation is not automatic, so each patient must receive approval.

The Medicare GLP-1 Bridge began on July 1, 2026. CMS currently plans to continue the program through December 31, 2027. During this period, eligible patients may receive approved GLP-1 weight management medications at a reduced cost. Program dates, medications, and requirements may change based on future CMS guidance.

Patients must have eligible Medicare Part D coverage and meet the clinical requirements set by CMS. Eligibility may depend on the patient’s body mass index, medical history, related health conditions, and reason for treatment. A qualified healthcare provider must prescribe the medication and confirm that the patient meets the program criteria. Not every Medicare beneficiary who wants a GLP-1 medication will qualify.

Eligible patients generally pay a $50 copay for a covered 28-day or 30-day supply. The standard Medicare Part D deductible does not apply to medications received through the Bridge program. However, the payment does not count toward the patient’s Part D out-of-pocket costs. Patients should confirm their exact cost and coverage before filling the prescription.

Yes. The prescribing healthcare provider must submit a prior authorization request that confirms the patient meets the Medicare GLP-1 Bridge requirements. CMS reviews the patient’s insurance status, medical information, and reason for treatment before approving coverage. 

Patients cannot enroll or receive approval without help from a qualified medical provider. An approved prescription may still require coordination with a participating pharmacy.

No. The Medicare GLP-1 Bridge only covers specific medications, doses, and formulations approved by CMS for the program. Coverage may also depend on why the medication was prescribed, since some GLP-1 drugs already have Medicare Part D coverage for certain conditions. 

Patients should not assume that every brand or form will qualify. The prescribing physician can help identify an appropriate covered option.

Yes. Medicare covers CCM, RPM, and RTM programs. Telehealth visit reimbursements are covered by insurance plans may vary depending on CMS updates but we help you navigate all options.

Yes. Caregivers can have secure access to the patient portal to view updates, patient’s vitals, appointments, and care plans.
