- Chronic disease management (e.g., diabetes, hypertension, heart disease)
- GLP-1 medication prescription and medications adjustments for weight management and diabetes treatment
- Referrals and coordination of services, including:
- Home health care services (nursing, physical therapy, occupational therapy)
- Durable Medical Equipment (DME) procurement (wheelchairs, oxygen tanks, hospital beds)
- Home modifications for safety and accessibility (ramps, stairlifts, grab bars)
- Managed Long-Term Care (MLTC) enrollment and support
- Palliative and hospice care coordination
